WISDOM ACADEMY LIABILITY WAIVER & RELEASE AGREEMENT 1. RELEASE OF LIABILITY: I hereby release, waive, discharge, and covenant not to sue Wisdom Academy, its officers, employees, agents, and representatives from any and all liability, claims, demands, actions, and causes of action whatsoever arising out of or related to any loss, damage, or injury, including death, that may be sustained by me while participating in activities at the fitness center and sports gym facilities. 2. ASSUMPTION OF RISK: I acknowledge that physical activities involve inherent risks including, but not limited to, serious injury, permanent disability, and death. I voluntarily assume all risks, both known and unknown, associated with my participation in fitness activities, sports activities, and use of equipment at Wisdom Academy facilities. 3. MEDICAL TREATMENT: I authorize Wisdom Academy staff to secure emergency medical treatment if necessary. I agree to be financially responsible for any medical or emergency costs incurred. 4. USE PERMISSION: I grant Wisdom Academy permission to use photographs, videos, or other media of me taken during activities for promotional purposes without compensation. 5. INDEMNIFICATION: I agree to indemnify and hold harmless Wisdom Academy from any loss, liability, damage, or cost that may arise from my participation in activities at the facility. 6. SEVERABILITY: If any portion of this agreement is found to be void or unenforceable, the remaining portions shall remain in full force and effect. 7. ARBITRATION: Any disputes arising from this agreement shall be resolved through binding arbitration in accordance with the rules of the American Arbitration Association. 8. MINOR RELEASE: For participants under 18 years of age, I certify that I am the parent or legal guardian and have the authority to enter into this waiver on behalf of the minor. I HAVE READ THIS RELEASE AND WAIVER OF LIABILITY, ASSUMPTION OF RISK, AND INDEMNITY AGREEMENT, FULLY UNDERSTAND ITS TERMS, AND UNDERSTAND THAT I HAVE GIVEN UP SUBSTANTIAL RIGHTS BY SIGNING IT. I SIGN IT FREELY AND VOLUNTARILY WITHOUT ANY INDUCEMENT.
